Aplicación desarrollada por la Sociedad Española de Cardiología (SEC) para ofrecer recomendaciones prácticas a los facultativos que asisten a pacientes con tratamiento antitrombótico ante una cirugía o procedimiento intervencionista. La app basa su algoritmo en el documento de consenso “Manejo perioperatorio y periprocedimiento del tratamiento antitrombótico” (Rev Esp Cardiol. 2018;71:553-64), coordinado por el Grupo de Trabajo de Trombosis Cardiovascular de la SEC y certificado por un amplio número de sociedades científicas que participan en el proceso asistencial del paciente, con el fin de homogeneizar la práctica clínica diaria.

Cada vez son más frecuentes los procedimientos quirúrgicos o intervencionistas en pacientes anticoagulados y antiagregados que pueden requerir la interrupción del tratamiento crónico antitrombótico. La decisión de retirar o mantener dicho tratamiento está determinada por el riesgo trombótico o por el hemorrágico y la interacción entre esos dos factores.

QxAApp ofrece a los profesionales, de una forma práctica y sencilla, respuestas a cuestiones como si es necesario suspender la anticoagulación y, en tal caso, cuándo; si es necesaria una terapia puente, cuándo y cómo reintroducir la anticoagulación o la antiagregación; cómo proceder ante una cirugía urgente, etc.

La aplicación se articula a través de un test en diferentes pasos, sin necesidad de registro y sin guardar la información que consulta para deducir la recomendación:

• Tipo de tratamiento y fármacos. El médico usuario indica qué tratamiento está siguiendo el paciente, si anticoagulante o antiagregante, y selecciona los fármacos. En función de la selección, la app identificará el riesgo trombótico y hemorrágico del paciente, clasificándolo como “alto”, “medio” o “bajo”.

• Cardiopatía y características del paciente. A continuación se detalla el tipo de cardiopatía, las características del paciente y sus riesgos.

• Tipo de intervención y riesgo hemorrágico. La aplicación da a elegir entre diferentes tipos de intervención quirúrgica a la que será sometido el paciente entre cinco categorías.

• Resumen y conclusiones. La app indica con cuantos días de antelación a la intervención se recomienda al paciente tomarse la última dosis de fármacos, si se considera necesaria una terapia puente y, por último, cuándo debería introducirse de nuevo el tratamiento tras la intervención.

Application developed by the Spanish Society of Cardiology (SEC) to offer practical recommendations to physicians who assist patients with antithrombotic treatment before a surgery or interventional procedure. The app bases its algorithm on the consensus document «Perioperative management and periprocedimiento of antithrombotic treatment» (Rev Esp Cardiol. 2018; 71: 553-64), coordinated by the Working Group on Cardiovascular Thrombosis of the SEC and certified by a broad number of scientific societies that participate in the patient’s care process, in order to standardize daily clinical practice.

Surgical or interventional procedures are increasingly common in anticoagulated and antiaggregated patients that may require the interruption of chronic antithrombotic treatment. The decision to withdraw or maintain such treatment is determined by the risk of thrombosis or hemorrhagic risk and the interaction between these two factors.

QxAApp offers professionals, in a practical and simple way, answers to questions such as whether it is necessary to stop anticoagulation and, in such a case, when; if bridging therapy is necessary, when and how to reintroduce anticoagulation or antiplatelet therapy; how to proceed before an urgent surgery, etc.

The application is articulated through a test in different steps, without needing registration and without saving the information that you consult to deduce the recommendation:

• Type of treatment and drugs. The user doctor indicates which treatment the patient is following, whether anticoagulant or antiaggregant, and selects the drugs. Depending on the selection, the app will identify the patient’s thrombotic and hemorrhagic risk, classifying it as «high», «medium» or «low».

• Heart disease and patient characteristics. The type of heart disease, the patient’s characteristics and their risks are detailed below.

• Type of intervention and hemorrhagic risk. The application gives a choice between different types of surgical intervention to which the patient will be submitted among five categories.

• Summary and conclusions. The app indicates how many days before the intervention the patient is recommended to take the last dose of drugs, if a bridge therapy is considered necessary and, finally, when it should be introduced. new the treatment after the intervention.
